AstraZeneca Plc (NASDAQ:<a class="ticker" href="https://www.benzinga.com/stock/AZN#NASDAQ">AZN</a>) on Monday released topline data from the DESTINY-Breast09 Phase 3 trial of&nbsp;Enhertu&nbsp;(5.4mg/kg) either alone or in combination with pertuzumab versus standard of care THP (a taxane [docetaxel or paclitaxel], trastuzumab, and pertuzumab) as a first-line treatment in patients with HER2-positive <a target="_blank" href="https://www.benzinga.com/general/health-care" rel="noreferrer noopener">metastatic breast cancer</a>.
High-level results from a planned interim analysis showed Enhertu pertuzumab combination demonstrated a statistically significant and clinically meaningful improvement in progression-free <a target="_blank" href="https://www.benzinga.com/pressreleases/25/04/b44891999/enhertu-plus-pertuzumab-demonstrated-highly-statistically-significant-and-clinically-meaningful-im" rel="noreferrer noopener">survival (PFS) compared to THP</a>.

The PFS improvement was seen across all pre-specified patient subgroups with&nbsp;Enhertu&nbsp;in&nbsp;combination with pertuzumab.
The key secondary endpoint of overall survival (OS) was not mature at this planned interim analysis; however, interim OS data showed an early trend favoring the&nbsp;Enhertu&nbsp;combination compared with&nbsp;THP.
The second arm assessing&nbsp;Enhertu&nbsp;monotherapy versus THP remains blinded to patients and investigators and will continue to the final PFS analysis.
HER2-positive metastatic breast cancer is an aggressive disease driven by overexpression or amplification of HER2, affecting 15-20% of patients with metastatic breast cancer.
While HER2-targeted therapies have improved outcomes, prognosis remains poor, with most patients experiencing disease progression within two years of 1st-line treatment with THP, which has been the standard of care for more than a decade.
Further, approximately one in three patients never go on to receive treatment following 1st-line therapy due to disease progression or death.
The safety profile of&nbsp;Enhertu&nbsp;in combination with pertuzumab was consistent with the known profiles of each therapy.
Enhertu&nbsp;is a specifically engineered HER2-directed DXd antibody-drug conjugate (ADC) discovered by Daiichi Sankyo (OTC:<a class="ticker" href="https://www.benzinga.com/stock/DSNKY#OTC">DSNKY</a>) (OTC:<a class="ticker" href="https://www.benzinga.com/stock/DSKYF#OTC">DSKYF</a>) and being jointly developed and commercialized by AstraZeneca and Daiichi Sankyo.
Data from the combination arm of DESTINY-Breast09 will be presented at an upcoming medical meeting and shared with regulatory authorities.
Enhertu is already approved in more than 75 countries as the 2nd-line treatment for patients with HER2-positive breast cancer based on the results from the&nbsp;DESTINY-Breast03&nbsp;trial.

In the trial,&nbsp;Enhertu&nbsp;showed a 36% reduction in the risk of disease progression or death versus chemotherapy in the overall trial population of patients with chemotherapy-naïve HER2-low or HER2-ultralow metastatic breast cancer.
A median progression-free survival (PFS) of 13.2 months was seen in patients randomized to&nbsp;Enhertu&nbsp;compared to 8.1 months in those randomized to chemotherapy.
The confirmed objective response rate (ORR) in the overall trial population was 62.6% for&nbsp;Enhertu&nbsp;versus 34.4% for chemotherapy.
In an exploratory analysis of patients with HER2-ultralow expression,&nbsp;results were seen to be consistent between patients with HER2-low expression and HER2-ultralow expression.&nbsp;&nbsp;
Following this approval for&nbsp;Enhertu&nbsp;in the U.S., AstraZeneca is due $175 million as a milestone payment to Daiichi Sankyo.
Daiichi Sankyo recognizes Enhertu sales in the U.S. AstraZeneca reports its share of gross profit margin from&nbsp;Enhertu sales in the U.S. as Alliance Revenue in the company&#8217;s financial statements.

On Friday, the FDA accepted and granted Priority Review AstraZeneca Plc's (NASDAQ:<a class="ticker" href="https://www.benzinga.com/stock/AZN#NASDAQ">AZN</a>) supplemental Biologics License Application (sBLA) for&nbsp;Imfinzi&nbsp;(durvalumab) for patients with muscle-invasive bladder cancer (MIBC).
The Prescription Drug User Fee Act date, the FDA action date for their regulatory decision, is anticipated during the second quarter of 2025.
The supplement application is <a target="_blank" href="https://www.astrazeneca.com/media-centre/press-releases/2024/imfinzi-granted-priority-review-in-the-us-for-patients-with-muscle-invasive-bladder-cancer.html" rel="noreferrer noopener">based</a> on data from the NIAGARA Phase 3 trial.

In the trial, patients were treated with&nbsp;Imfinzi&nbsp;in combination with neoadjuvant chemotherapy before radical cystectomy, followed by&nbsp;Imfinzi&nbsp;as adjuvant monotherapy or neoadjuvant chemotherapy before radical cystectomy.
In a planned interim analysis, perioperative&nbsp;Imfinzi&nbsp;demonstrated a 32% reduction in the risk of disease progression, recurrence, not undergoing surgery, or death versus neoadjuvant chemotherapy with radical cystectomy alone.
The estimated median event-free survival (EFS) for the&nbsp;Imfinzi&nbsp;arm has not yet been reached, versus 46.1 months for the comparator arm. An estimated 67.8% of patients treated with the&nbsp;Imfinzi&nbsp;regimen were event-free at two years, compared to 59.8% in the comparator arm.
Results from the key secondary endpoint of overall survival (OS) showed that the&nbsp;Imfinzi&nbsp;perioperative regimen reduced the risk of death by 25% versus neoadjuvant chemotherapy with radical cystectomy.
Median survival was not yet reached for either arm. An estimated 82.2% of patients treated with the&nbsp;Imfinzi&nbsp;regimen were alive at two years, compared to 75.2% in the comparator arm.
On Thursday, a&nbsp;pooled analysis of the&nbsp;TROPION-Lung05 phase 2 and the&nbsp;TROPION-Lung01 phase 3 trials <a target="_blank" href="https://www.businesswire.com/news/home/20241205215995/en/Datopotamab-Deruxtecan-Demonstrated-Meaningful-Clinical-Activity-in-Patients-with-Previously-Treated-Advanced-EGFR-Mutated-Non-Small-Cell-Lung-Cancer-in-TROPION-Lung05-and-TROPION-Lung01-Pooled-Analysis" rel="noreferrer noopener">showed</a> datopotamab deruxtecan (Dato-DXd) demonstrated clinically meaningful tumor response in patients with previously treated advanced or metastatic EGFR-mutated non-small cell lung cancer (NSCLC).
These data and progression-free and overall survival results from the analysis&nbsp;were presented at the 2024 ESMO Asia Congress.
Daiichi Sankyo (OTC:<a class="ticker" href="https://www.benzinga.com/stock/DSKYF#OTC">DSKYF</a>) (OTC:<a class="ticker" href="https://www.benzinga.com/stock/DSNKY#OTC">DSNKY</a>) and AstraZeneca's datopotamab deruxtecan showed a 42.7% objective response rate in previously treated patients.
Five (4.3%) complete responses, 45 (38.5%) partial responses, and 48 (41.0%) cases of stable disease were observed.
The median duration of response was 7.0 months, and the disease control rate was 86.3%. Median progression-free survival was 5.8 months, and median overall survival was 15.6 months.

The <a target="_blank" href="https://www.astrazeneca.com/media-centre/press-releases/2024/datopotamab-deruxtecan-final-overall-survival-results-reported-in-patients-with-metastatic-hr-positive-her2-low-or-negative-breast-cancer-in-tropion-breast01-phase-iii-trial.html" rel="noreferrer noopener">study</a> did not achieve statistical significance in the final overall survival (OS) analysis in patients with inoperable or metastatic hormone receptor (HR)-positive, HER2-low or negative breast cancer previously treated with endocrine-based therapy and at least one systemic therapy.
The study previously met the dual primary endpoint of progression-free survival.

The safety profile of datopotamab deruxtecan was consistent with that observed in the previous analysis, including lower rates of Grade 3 or higher treatment-related adverse events compared to chemotherapy, and no new safety concerns were identified.
All-grade interstitial lung disease (ILD) rates remained low, and no new Grade 3 or higher ILD events were observed.
With multiple antibody-drug conjugates (ADCs) approved during the trial, including&nbsp;Enhertu (trastuzumab deruxtecan), subsequent treatment following patients&#8217; disease progression or treatment discontinuation is likely to have affected survival results.

Earlier this month, AstraZeneca&nbsp;shared detailed results from the TROPION-Lung01 Phase 3 of datopotamab deruxtecan (Dato-DXd) compared to docetaxel, the current standard of care chemotherapy for locally advanced or metastatic nonsquamous non-small cell lung cancer (NSCLC) treated with at least&nbsp;<a target="_blank" href="https://www.benzinga.com/general/biotech/24/09/40791315/astrazenecas-investigational-cancer-drug-disappoints-in-late-stage-lung-cancer-trial" rel="noreferrer noopener">one prior line of therapy</a>.
In the overall trial population, OS results numerically favored datopotamab deruxtecan compared to docetaxel (12.9 vs. 11.8 months) but did not reach statistical significance.
Friday, the FDA <a target="_blank" href="https://www.astrazeneca.com/media-centre/press-releases/2024/flumist-approved-for-self-administration-in-the-us.html" rel="noreferrer noopener">approved</a> AstraZeneca&#8217;s FluMist as the only self-administered influenza vaccine.&nbsp;
FluMist, a needle-free nasal spray, was approved for self-administration by adults up to 49 years of age or for parent/caregiver administration to individuals 2-17 years of age.
Once available, individuals 18 and older can have FluMist delivered directly to their homes via FluMist Home.

Latest Trial Setback For AstraZeneca's Dato-DXd, The Drug Fails To Extend Lives Of Pretreated Breast Cancer Patients

Monday, AstraZeneca&nbsp;Plc (NASDAQ:<a class="ticker" href="https://www.benzinga.com/stock/AZN#NASDAQ">AZN</a>) shared detailed results from the TROPION-Lung01 Phase 3 of datopotamab deruxtecan (Dato-DXd) compared to docetaxel, the current standard of care chemotherapy for locally advanced or metastatic nonsquamous non-small cell lung cancer (NSCLC) treated with at least <a target="_blank" href="https://www.benzinga.com/pressreleases/24/09/b40779283/datopotamab-deruxtecan-showed-median-overall-survival-of-14-6-months-in-patients-with-advanced-non" rel="noreferrer noopener">one prior line of therapy</a>.
Detailed results showed a clinically meaningful trend toward improving overall survival (OS) with datopotamab deruxtecan (Dato-DXd) compared to docetaxel.

Dato-DXd is an engineered TROP2-directed DXd antibody-drug conjugate discovered by Daiichi Sankyo Ltd (OTC:<a class="ticker" href="https://www.benzinga.com/stock/DSKYF#OTC">DSKYF</a>) (OTC:<a class="ticker" href="https://www.benzinga.com/stock/DSNKY#OTC">DSNKY</a>) and being jointly developed by AstraZeneca and Daiichi Sankyo.
In the overall trial population, OS results numerically favored datopotamab deruxtecan compared to docetaxel (12.9 vs. 11.8 months) but did not reach statistical significance (hazard ratio [HR] 0.94; p=0.530).
In the prespecified subgroup of patients with nonsquamous NSCLC, datopotamab deruxtecan showed a 2.3-month improvement in OS compared to docetaxel (14.6 vs. 12.3 months).
The safety profile of datopotamab deruxtecan in TROPION-Lung01 was consistent with the previous analysis, including lower rates of dose reduction (20%, 30%) and discontinuation (8%, 12%) due to adverse events compared to docetaxel.
At the World Conference on Lung Cancer (WCLC), the British pharma giant shared results from the NeoCOAST-2 Phase 2 platform trial of Imfinzi&nbsp;(durvalumab) in multiple combinations, before and after surgery, in patients with early-stage (Stage IIA–IIIB) resectable NSCLC.
Preliminary results from the trial arm testing neoadjuvant Imfinzi plus datopotamab deruxtecan and carboplatin demonstrated a pathological complete response rate of 34.1% and a major pathological response rate of 65.9%.
Recently, the FDA approved&nbsp;AstraZeneca&#8217;s Imfinzi&nbsp;(durvalumab) in combination with chemotherapy for adult patients with resectable early-stage NSCLC and no known epidermal growth factor receptor mutations or anaplastic&nbsp;<a target="_blank" href="https://www.benzinga.com/general/biotech/24/08/40416174/fda-approves-astrazenecas-blockbuster-cancer-drug-imfinzi-in-combination-with-chemo-for-non-small" rel="noreferrer noopener">lymphoma kinase rearrangements</a>.

On Wednesday, the FDA issued a Complete Response Letter (CRL) for the Biologics License Application (BLA) seeking accelerated approval of Daiichi Sankyo (OTC:<a class="ticker" href="https://www.benzinga.com/stock/DSKYF#OTC">DSKYF</a>) (OTC:<a class="ticker" href="https://www.benzinga.com/stock/DSNKY#OTC">DSNKY</a>) and Merck &amp; Co Inc&#8217;s (NYSE:<a class="ticker" href="https://www.benzinga.com/stock/MRK#NYSE">MRK</a>)&nbsp;patritumab deruxtecan (HER3-DXd) for adult patients with locally advanced or metastatic EGFR-mutated non-small cell lung cancer (NSCLC) previously treated with two or <a target="_blank" href="https://www.benzinga.com/pressreleases/24/06/b39515260/patritumab-deruxtecan-bla-submission-receives-complete-response-letter-from-fda-due-to-inspection-" rel="noreferrer noopener">more systemic therapies</a>.
The CRL results from findings about an inspection of a third-party manufacturing facility.
The CRL did not identify any issues with the efficacy or safety data submitted.
Patritumab deruxtecan is a specifically engineered potential first-in-class HER3-directed DXd antibody-drug conjugate (ADC) discovered by Daiichi Sankyo and being jointly developed by Daiichi Sankyo and Merck.
In December, the FDA accepted and granted Priority Review to the Biologics License Application (BLA) for patritumab deruxtecan (HER3-DXd).
If approved, patritumab deruxtecan would be a first-in-class HER3-directed DXd antibody-drug conjugate <a target="_blank" href="https://www.benzinga.com/pressreleases/23/12/b36360381/patritumab-deruxtecan-granted-priority-review-in-the-u-s-for-certain-patients-with-previously-trea" rel="noreferrer noopener">for these patients</a>.
&#8220;We will work closely with the FDA and the third-party manufacturer to address the feedback as quickly as possible in order to bring the first HER3-directed medicine to patients with previously-treated EGFR-mutated non-small cell lung cancer,&#8221; said Ken Takeshita, MD, Global Head, R&amp;D, Daiichi Sankyo. &#8220;We remain confident in the ability to develop this medicine to its full potential.&#8221;
The BLA is based on the primary results from the&nbsp;HERTHENA-Lung01&nbsp;pivotal phase 2 trial.
In HERTHENA-Lung01, patritumab deruxtecan was studied in 225 patients, which demonstrated an objective response rate (ORR) of 29.8%, including one complete response and 66 partial responses.
The median duration of response (DoR) was 6.4 months.

FDA Declines To Approve Merck-Daiichi Sankyo Partnered Lung Cancer Drug

No new interstitial lung disease events of any grade were adjudicated as drug-related.

A statistically significant and clinically meaningful improvement in PFS was also observed in the overall trial population (patients with HER2-low and HER2-ultralow [defined as IHC 0 with membrane staining; IHC &gt;0&lt;1+] metastatic breast cancer).&nbsp;

A prespecified subgroup analysis showed that clinically meaningful improvement was consistent between patients with HER2-low and HER2-ultralow&nbsp;expression.
Overall survival (OS) data were not mature at the time of the analysis. However, Enhertu showed an early trend towards an OS improvement. It was compared to standard-of-care chemotherapy in patients with HER2-low metastatic breast cancer and the overall trial population. 
The trial will continue to assess OS and other secondary endpoints further.

Truqap in combination with Faslodex reduced the risk of disease progression or death by 50%. Compare that to Faslodex alone in patients with tumors harboring PI3K, AKT, or PTEN alterations (based on a hazard ratio of 0.50, p=&lt;0.001; median PFS 7.3 versus 3.1 months).
